The HMH Accountable Care Organization

The HMH ACO is Hackensack Meridian Health’s ACO comprised of more than 1,300 of New Jersey’s top physicians - working in conjunction with Hackensack Meridian’s hospitals.

As an ACO, our providers agree to be accountable for the quality, cost and overall care of groups of Medicare beneficiaries. The HMH ACO offers systems that help physicians coordinate healthcare through improved communication, evaluation and measurement. This approach increases efficiency for patients and physicians alike.

The organization supports its doctors in caring for their patients by ensuring physicians have the most current health information available. This means physician communication is better, patients avoid duplicate tests, and care is coordinated and streamlined, providing every patient a better healthcare experience.

Collectively, the physicians participating in the HMH ACO have been serving New Jersey residents for decades, providing quality and compassionate medical care to improve the healthcare outcomes in the surrounding communities.

Care Coordination Among HMH Healthcare Providers

The HMH ACO is working with area providers to help them focus on your unique healthcare needs. Together, we are providing better follow-up care and creating more cohesive care teams out of doctors, nurses, specialists, technicians, and social workers.

Managing Care & Resource Referrals

The HMH ACO can assist participating physicians in keeping patients as healthy as possible and support the reduction of unnecessary (expensive) interventions like duplicate tests, emergency department visits, hospitalizations, and procedures. We can also offer other resources, such as prescription assistance and community support programs – as part of your care plan.

Chronic Health Support Services

Our HMH ACO health partners can better assist patients with chronic health conditions and develop a care plan to keep you moving toward better health outcomes.

Post-Hospital Care Assistance

As HMH ACO patients return home from a hospital stay, our health partners are available to ensure a smooth transition with the appropriate follow-up care plans and also support you with your healthy living goals.
